What is an Ankle Sprain?

An ankle sprain, commonly known as “spraining the ankle,” is an injury caused by the sudden abnormal twisting or stretching of the ankle joint. For example, stepping onto an uneven surface can cause the ankle to invert rapidly, leading to stretching or even tearing of the ankle ligaments.

Everyone is likely to experience ankle sprains multiple times in their lifetime. In fact, according to surveys, approximately 25,000 people worldwide experience an ankle sprain each day. Ankle sprains occur not only during sports but also during daily activities, especially as modern individuals often walk while using their phones. Any uneven or sloped surface can easily lead to an accidental sprain. Mild sprains generally do not require treatment; adequate rest and protection of the injured area can lead to quick self-healing. Conversely, more severe sprains should be examined and treated by a physical therapist to avoid improper handling of the injury, which can lead to a series of compensatory body issues and long-term stability problems.

Causes of Ankle Sprains

  • Stepping onto an uneven surface while walking
  • Sudden turns or unstable landings during sports
  • Wearing inappropriate shoes or having loose shoelaces
  • Wearing high heels, which can cause instability
  • Lack of sufficient muscle support for the joints
  • Previous ankle injuries, resulting in poorer joint stability and higher risk of recurrence

Common Symptoms of Ankle Sprains

  • Pain: Intense pain around the ankle joint, especially noticeable when pressed
  • Swelling: Swelling in the injured area, which may affect walking or wearing shoes
  • Bruising and Discoloration: Skin discoloration near the ankle due to bruising
  • Restricted Movement: Reduced range of motion in the ankle joint, unable to rotate or bear pressure normally
  • Weight-Bearing Difficulties: Feeling that the ankle joint cannot support body weight when walking or standing
  • Joint Looseness: Feeling of instability in the ankle, making it easy to sprain again

Types and Severity of Ankle Sprains

Most ankle sprains are caused by external forces. As long as the injury is accurately assessed and properly treated, long-term complications are uncommon. We aim to share some clinical knowledge here to help you better understand your underlying issues and trust our professional expertise.

內翻性拗柴扭傷

The most common type of sprain, accounting for about 80% of all cases. This type of injury is caused by the ankle excessively and rapidly inverting, leading to the stretching of the three lateral ligaments. The most easily torn ligament is the Anterior Talofibular Ligament.

外翻性拗柴扭傷

Caused by the ankle excessively everting, leading to the stretching of the three medial ligaments. Since the fibula's ankle position on the outer side of the calf is lower than the inner side, the degree of ankle eversion is generally more limited. Therefore, eversion sprains are less common and typically occur when landing on an uneven surface or making a sudden side stop.

高位踝關節拗柴扭傷

A more severe type of ankle sprain involving the distal joints of the tibia and fibula, leading to ligament stretching. This injury is often rotational; when the foot is planted on the ground but the lower limb twists rapidly, it can stretch the ligaments in this area, usually resulting in significant swelling in higher areas.

These injuries have the potential to worsen when the joint bears weight, causing separation of the tibial and fibular joints. Therefore, early assessment of the injury and the use of ankle braces are necessary, and the recovery period is longer than that of typical inversion or eversion sprains.

骨折

Ankle sprains can lead to fractures. When the twisting force is excessive or involves impact, the fibula or even the tibia may fracture. It is recommended to seek professional examination if there is significant swelling, pain, or bruising around the bone area.

Avulsion Fractures occur when the bone where the ligament attaches is partially torn away due to forceful pulling. These types of injuries usually require X-rays for further confirmation. Most do not heal on their own but do not require surgical intervention either. After a period of rest, the joint can regain stability.

慢性腳踝關節不穩(慣性拗柴)

This type of injury generally results from moderate to severe ligament tears, causing the ankle joint to lose strong ligament support and become unstable. Symptoms include poor balance, repeated minor sprains on flat surfaces, occasional swelling and pain in the joint.

Clinically, physical therapists will assess ligament laxity and perform a series of functional and muscle strength tests to determine the underlying issues. Continuous training to improve ankle stability is recommended to alleviate symptoms and prevent the problem from worsening. Only a small percentage of patients require surgery.

一級拗柴

A mild sprain refers to the stretching of the ankle ligaments without significant tearing or severe damage. You may experience localized pain and mild swelling, but the range of motion typically remains intact, and the ankle can still bear some weight stably.

  • Treatment Recommendations

Following the latest sports injury guidelines (Peace & Love) can effectively alleviate symptoms. It is recommended to reduce walking volume and speed in daily life, wear compression socks, and keep the ankle active to maintain blood circulation without necessarily wearing an ankle brace.

  • Recovery Time:
Typically recovers within two to four weeks.
二級拗柴

A moderate sprain involves partial tearing of the ligaments, usually resulting in more severe swelling and pain, along with bruising. Unstable walking conditions occur. This type of injury mostly occurs during high-intensity sports or intense collisions, such as collisions between rugby players.

  • Treatment Recommendations:In the initial phase, it is recommended to wear an ankle brace to stabilize the ankle, combined with physical therapy to accelerate recovery: high-intensity laser therapy, focused shockwave therapy, and pulsed magnetic field (PMF) physical therapy. Physical therapists will also plan corresponding exercise training to enhance balance and ankle strength. Due to more severe swelling and pain, weight-bearing and walking posture are typically avoided. Patients need to relearn how to distribute weight while walking, calf muscle strength, and single-leg balance.
  • Recovery Time:
Recovery takes three months or longer.
三級拗柴

The most severe degree of ligament sprain can result in a complete tear. If you hear a "pop" sound during the sprain or even experience joint dislocation, it may belong to this category. Severe sprains cause intense pain, inability to stand or walk normally, and significant swelling and bruising around the ankle, which can last for an extended period.

  • Treatment Recommendations:
Severe sprains typically require comprehensive treatment plans. Physical therapists first check for possible fractures and the extent of ligament tears, recommend wearing an ankle brace, and may even use crutches to assist walking. After pain and swelling improve, balance and surrounding muscle strength exercises can be intensified. Proper handling of these injuries is crucial, and patients must follow instructions and remain patient. Premature return to high-intensity activities can affect healing and cause more complications.
  • Recovery Time:
Recovery takes three months or longer.

Diagnosing Ankle Sprains

The first step in treating an ankle sprain is precise diagnosis and thorough examination to confirm the injury. Physical therapists will assess the actual condition of the problem, rule out other structural issues, and design the most suitable treatment plan based on the patient’s needs, helping patients quickly alleviate pain and restore function.

One-on-One Consultation

Ankle sprain issues are not complicated. The consultation aims to differentiate the type and severity of the injury, which is especially crucial for patients with chronic joint instability. Through professional physical therapists’ analysis, understanding each patient’s underlying factors, and combining rehabilitation exercises with adjustments to exercise habits, the root cause can be thoroughly addressed.

臨床檢查

Clinical Examination

In addition to medical tests, clinical examinations are also an effective method for assessing ankle sprains. Physical therapists will test various movements and areas, observing the location of pain to determine your condition. These may include:

  • Specific Tests, such as the Anterior Drawer Test
  • Ankle Range of Motion Assessment
  • Strength Testing
  • Palpation to Determine the Sprain Location
  • Surrounding Joint and Fascia Assessment
MRI醫學檢測

Medical Testing

When a sprain persists or worsens, it is recommended to seek assistance from doctors and professional physical therapists. Severe and unusual sprain issues may require imaging diagnostic tools, including MRI, X-rays, and ultrasound, to rule out fractures or other problems.

How Does OneEssential Treat Your Ankle Sprain?

Focused Shockwave Therapy

“Electroacoustic shockwave” technology uses high-energy shockwaves to penetrate the skin deeply and reach the affected areas of the ankle ligaments and other soft tissues. This treatment stimulates cellular repair, accelerates recovery, and quickly alleviates pain, even aiding in the healing of chronic injuries.

High-Energy Laser Therapy

Laser therapy can stimulate local blood circulation and promote lymphatic return in the injured ankle area, effectively reducing swelling and discomfort. Combined with biostimulation and photomechanical technology, it promotes soft tissue repair.

Manual Therapy

Joint mobilization techniques target the tightened ankle joint after a sprain, passively improving mobility and reducing discomfort. Calf muscles may become tight due to sprain strain or compensation, and appropriate manual therapy can relax muscles and improve symptoms.

Acupuncture Therapy

Dry needling, also known as modern acupuncture, is an innovative treatment method for musculoskeletal issues. Physical therapists insert fine needles into the injured ankle joint and surrounding calf muscles to promote local blood circulation and healing responses, effectively relieving pain caused by sprains.

Frequently Asked Questions about Ankle Sprains

Both icing and heating are effective methods for symptom relief and can be chosen based on different symptoms and purposes. However, these methods only alleviate symptoms and do not improve tendon injuries.

Icing: In the initial phase of a sprain (first 48 hours), there is usually significant swelling. Icing can temporarily constrict blood vessels and numb superficial nerves to achieve pain relief and reduce discomfort. It is recommended to ice for fifteen minutes and follow your physical therapist's guidance on joint movement.

Heating: After the acute phase, even if there is still swelling and pain, this may be due to the accumulation of fluid from inflammation. In this case, heating can promote blood circulation, accelerate the dissipation of bruising, and relax tight surrounding muscles. It is recommended to soak your feet in hot water for twenty minutes.

Pain patches (such as Salonpas) may have some effect on mild sprains as they contain anti-inflammatory and pain-relieving ingredients, providing temporary pain relief. However, for moderate or severe sprains, the effectiveness of pain patches is limited. To address the root cause, we recommend focusing on physical therapy, as pain patches can only serve as auxiliary tools to alleviate pain.

The notion of avoiding getting wet after an ankle sprain originates from traditional Chinese medicine's concept of "dampness." Traditional Chinese medicine believes that getting wet, especially with cold water, after an injury can increase dampness and prolong recovery time. However, from a modern medical perspective, it is generally acceptable to get wet after an ankle sprain.

During the acute phase of inflammation after a sprain, it is recommended to reduce alcohol consumption. Alcohol has a vasodilating effect, which can exacerbate swelling and inhibit the body's immune response, delaying the repair of injured tissues.

Frequently Asked Questions about Ankle Sprain Physical Therapy

Certainly effective. OneEssential's physical therapists follow scientifically proven treatment and training methods to address tendon issues. They use various instruments and acupuncture to improve symptoms and guide you through different strengthening exercises at appropriate stages. Whether you are an athlete or not, encountering tendon problems requires proper training to cure the issue and prevent recurrence.

Elastic exercise tapes (Kinesio tape) are commonly used by physical therapists as a treatment method. However, current scientific findings indicate that these auxiliary tools have limited effects. Besides providing psychological confidence to patients, they may somewhat alleviate pain but cannot genuinely reduce the load on the tendon during exercise. In simple terms, tapes can improve some symptoms but cannot protect the tendon or prevent its deterioration.

OneEssential's physical therapy services charge $1,400 for the initial 60-minute consultation, which includes all examinations and treatments without any additional fees.

The service is entirely handled by registered physical therapists, including clinical examinations and rehabilitation training. Treatments for ankle sprains include manual therapy, acupuncture, shockwave therapy, etc., and will be adjusted based on each consultation and examination result to provide the most safe and reliable service.
